In the main UpRaise menu, click |
on forms >> template tab >> create new template. |
Select Survey type from the pop-up and |
click next. |
 In the next step, select one of the available workflows. Right now two workflows are available for survey type of template. You can look at the flowchart to understand how different transitions are handled.

 After clicking on next user is brought to the Form Builder page where he can add different questions. Since only one person (reviewer) can respond to the questionnaire, there is only one tab while designing the template. |
