Click on Create new template as an UpRaise admin/super admin/user with sufficient permission to create template. Select Confidential review type from the pop-up and click next.
In the next step, select one of the available workflows. Right now two workflows are available for 'confidential review' type of template. You can look at the flowchart to understand how different transitions are handled. 
After clicking on 'next' user is brought to the 'form builder' page where he can add different questions.
Once questions are added, click on 'save' or 'save & next'Save or Save & Next. The template will be saved & can now be distributed. If clicked on save, then the template will be saved in templates listing for later use.
