In the main Employee Success main UpRaise menu, click on 'forms' & then click on 'create new template'. Select '360 review' type from the pop-up and click 'next'.
In the next step, select the workflow. You can look at the flowchart to understand how different transitions are handled. 
After clicking on 'next' user is brought to the 'form builder' page where he can add questions. In this type of review, one team member can be reviewed by multiple managers or peers and they can respond to the questionnaire in parallel.

Once questions are added, click on 'save' or 'save & next'. The template will be saved & can now be distributed. If clicked on save, then the template will be saved in templates listing.
