How to design a self & manager review template?

Navigate to the form builder 

Click on forms page in the main menu, then click on templates tab and then on create new template. Select Self & manager review template and click next.



In the next step, you can look at the workflow to understand how different transitions are handled. 



After clicking on next, user is brought to the form builder page where he can add questions to different tabs. These tabs are based on responsibility as well as visibility of different stakeholders involved (team member, reviewer & HR person).


Details of all the tabs

  • Team Member & Reviewer - Questions which should be answered by both, team member & reviewer are to be added here. When shared, visible to - team member, reviewer & HR person.

  • Team Member - Questions which are exclusively answered by the team member. When shared, visible to - team member, reviewer & HR person.

  • Reviewer - Questions which are exclusively answered by the reviewer. When shared, visible to - team member, reviewer & HR person.

  • Reviewer Remarks (optional) - Questions exclusively answered by the reviewer. In any case, these are not visible to the team member. Only reviewer & HR person can see.

  • HR Remarks (optional) - Questions exclusively answered by the HR person. In any case, these are visible only to the HR person. 

  • Note that, HR person is the UpRaise admin who distributes the template.

  • Once the template is distributed, even if super admin is not one of the stakeholders of that distribution, he/she will have access to it. Irrespective of whether the distribution is shared or not super admin will have access to all data of each distribution in the system.

  • It is not necessary to add questions in each tab.

    • Example-  In any case, if you think HR remarks are not so important or reviewers remarks are not part of your review process then you can simply avoid adding any question in those tabs. Only requirement is that - team member & reviewer tab should have at least one question that is to be answered by them.
