Form builder

Form builder is the place where you design the template using different question types and customizations. Whatever be the template type, available set of questions is same. Read this section for more details on question types.

Navigate to Forms >> Templates tab >> Create new template >> Select relevant template >> Next >> Select workflow >> Next

Based on selected template, tabs on form builder page are going to be different as explained below.

 

When you are using Team member assessment followed by Reviewer or Team member and Reviewer in parallel workflows, following tabs are available.

  • Team Member & Reviewer - Questions which should be answered by both, team member & reviewer are to be added here. When shared, visible to - team member, reviewer & HR person.

  • Team Member - Questions which are exclusively answered by the team member. When shared, visible to - team member, reviewer & HR person.

  • Reviewer - Questions which are exclusively answered by the reviewer. When shared, visible to - team member, reviewer & HR person.

  • Reviewer Remarks (optional) - Questions exclusively answered by the reviewer. In any case, these are not visible to the team member. Only reviewer & HR person can see.

  • HR Remarks (optional) - Questions exclusively answered by the HR person. In any case, these are visible only to the HR person. 

  • Note that, HR person is the UpRaise is the one who distributes the template.

  • It is not necessary to add questions in each tab.

    • Example-  In any case, if you think HR remarks are not so important or reviewers remarks are not a part of your review process then you can skip it. Only requirement is that - Team member & reviewer tab should have at least one question that is to be answered by them.

When you are using One-on-one discussions workflow, following tabs are available.

  • Team Member & Reviewer - Questions which should be answered by both, team member & reviewer are to be added here. When shared, visible to - team member, reviewer both.

  • Team Member - Questions which are exclusively answered by the team member. When shared, visible to - reviewer.

  • Reviewer - Questions which are exclusively answered by the reviewer. When shared, visible to - team member.

 

In this template, there is only one tab called Reviewer. All the questions added to this tab are common to team member (If self assessment is turned on) and other reviewers (Peers/Managers/Direct reports) added while distributing the template.

In this way, once you are on the form builder page, There are three tabs in left section.

  • Form settings

  • Add a field

  • Field settings

Form settings

On this tab, you need to add all form related details.

  • Name - This is your template name. Example - i) Semi annual performance review for team sales, ii) Probation review for software developers, iii) Confidential review before promotion for team leads, etc.

  • Description - This can be anything that describes the template well.

  • Other settings - If you check Show feedback tab & Show issues tab, these two tabs will appear in the form for end result to refer feedback reviewee has received over the time and the Jira issues they have worked on.

Add a field

Once you are done with Form settings, click on Add a field. As shown in the image above, there is a list of questions types/sections that you can add in the template.

Click on the question type you want to add. When you click on the question added to the right side/within different tabs of the template, system takes you to the Field settings tab to customize the question.

Field settings

This tab shows you all options to customize the information for each question. Read the last section on this page for more details on each question type & their customization.

Once you are done with entire template design. Click on Save so you can distribute the template later. Or click on Save & Next to distribute the template right away.

Cancel option will discard your template.

Possible Question types in UpRaise forms

When you are creating any type of template, UpRaise provides a set of question types that you can add within a template. Let us see what all types of questions are available.

For the purpose of example above image is taken with Self and manager template. Same list of question is available for all type of templates.

Import OKRs

Not a question type, however you want OKRs to be the part of your performance review process, import OKRs option is helpful. While creating a template, if admin adds this import OKRs option, reviewer of the form will have an opportunity to import team member's OKRs in that form and weight them so both team member and reviewer and assess those objectives on a scale that is selected while designing the form.

3 settings it has are, Opinion scale, Instructions, Value - Mandatory & add comment box, start a scale at one.

Import OKRs is applicable only for Self and manager review template.

Section start

Again, not a question type but an option to create sections-wise questionnaire. This option helps you to create structured form. These sections can be scored. For example you have created a form with sections based on skills, at the end you will get scores for each skill. It has 4 customizable settings - Section Title & Section Details, Value - Scores & weightage.

Single line text

As the name suggests, this question type is used when you wish to get single line answers.

Paragraph text

This question type is used when you expect an answers in 3-4 lines. It has 4 customizable settings as that of the single line text - Question, Default Value, Instructions, Value - Mandatory.

Date

This question type is quite simple to understand. When you are expect an answer to be some date, you can use this question type. Setting option are - Question, Default value, Instructions, Value - Mandatory.

Multiple choice

This question type is useful when answer is expected with multiple choice options. You can add multiple options of your choice. Numerical & text options are accepted. Settings for this question type are - Question, Choices, Instructions, Value - Mandatory, Add comment box, Score, Weightage.

Check box

When you expect an answer with more than one options from given choices, use check box question type. Customization option available in this question are - Questions, Choices, Instructions, Value - Mandatory & Add comment box.

Star ratings

When the answer is expected in a rating type. Each of the stars is associated with a label that is displayed while filling up the form. Customizable settings for this question type are - Question, Choices, Instructions, Mandatory, Value - Mandatory, Add comment box, Score & Weightage.

Dropdown

When the list of possible options are to be displayed in the form of a drop-down, use this question type. Settings option for this question type include - Question, Choices, Instructions, Values - Mandatory, Add comment box, score & Weightage.

Yes/No

If the expected answer is Yes/No then this question type can be used. Possible setting option for this question type are - Question, Instructions, Values - Mandatory, Add comment box.

Opinion scale

When answer is expected on a scale or range, use this question type. Settings to customize the question are - Question, Opinion scale (Range of numbers), Labels for the extreme values, Instructions, Values - Mandatory, Add Comment Box, Score & Weightage, Start scale at one (by default the range starts from 0, check this to start it from 1 instead).

You can mix and combine any type of questions from above list and design any given type of template. Once the template is saved, it can be used multiple times to send across groups/teams.
