When to use survey template?

Use this template to gather votes, and feedback from teams. There is always a situation when organizations want to run some voting poles to make decisions based on employee feedback.

This template has 2 different workflows. Assign to UpRaise admin will assign the form to admins at the end to evaluate response. Whereas the other workflow called Assign to Reviewver’s manager will assign the form the form to designated manager of the reviewer as per UpRaise company directory set up and then Manager decides to approve or disaprove the given response by reviewer.

Create

Navigate to Forms >> Templates >> Create new template >> Survey

Seleect the relevant workflow and click on Next. This lands on survey template builder.

Using Add a field and Fields settings tab, add any type of question available in the form builder. Read https://amoeboids.atlassian.net/wiki/spaces/UPFJ/pages/3931177056/Manage+Templates#Create. Form creation process is the same for every template, the difference is only for form users. For example, in this form user type will be reviewers only, there are no other types of reviewers, unlike other templates in UpRaise. Workflows

Like mentioned above there are two workflows for survey template.

Distribute

As explained in . The process of distribution is the same for all types of templates.

UpRaise allows to import an Excel file that contains the list of reviewers. Once can easily prepare the file and export directly instead of manually setting up users to distribute forms. Read .

Once the entire set up is done, one can save, schedule, or distribute the form. read .

View responses

On the view responses page, admin/HR (whoever distributes the form) can keep an eye on the form statuses. They may want to generate some reports by exporting forms or create user generate starts using UpRaise reports, all of this is possible on view responses page.

As shown in above image, there are three tabs on the view responses page.

  • Forms - Displays all the forms in the distribution

  • Reports - Generate report with the given distribution right away, instead of navigating to the reports section

  • Details - All the distribution details

Forms

  1. As shown in the above image, Forms tab is all about the forms and their information and actions. From this tab, the top char shows the statuses of the forms.

  2. Filters to search for specific set of users.

  3. Plus icon allows to add more user pairs in the ongoing distribution.

  4. Actions (…) beside plus sign has actions like Export to Excel, Start/stop accepting inputs, and Extend due date. These options work at the distribution level. Below that, provided is the list of users added to the form.

  5. Every form is displayed with its current status, scores, assignee, etc.

    1. Green pencil icon dictates that form is still accepting input. When form is closed, red pencil icon will be displayed and it won’t accept any input.

  6. Actions (…) has options like Export to Excel, Start/stop accepting input, Delete, Print. This will be available for each team member and work at user level only.

  7. View option to open and view individual form.

Reports

This tab has a two types of reports. Clicking on report generates a report for the current distribution. This is a handy way to generate a report instead of navigating to Reports section in the app. Read .

Details

This section is quite self-explanatory. This tab contains all the distribution details added while creating a distribution. A handy way to check the distribution setup.
