Confidential review forms

When to use Confidetial review template?

Use this template when any team member needs to review without their knowledge. The typical use case for this is the reviewing employee at the end of probation. There is always a situation when organizations want to review some employees without informing them.

This template has 3 different workflows. Assign to UpRaise admin will assign the form to admins at the end of the review process. Next workflow is Assign to Reviewver’s manager, in this the form is assigned to designated manager of the reviewer as per UpRaise company directory setup. And the third one is. Assign to UpRaise admin - Shared with team member.

Create

Navigate to Forms >> Templates >> Create >> Confidetial review

Using Add a field and Fields settings tab, add any type of question available in the form builder. Read Manage Templates | Create. Form creation process is the same for every template, the difference is only for form users. For example, in this form user type will be reviewers only, there are no other types of reviewers, unlike other templates in UpRaise. Also, read Types of questions that can added in templates form builder.

Workflows

There are three types of workflows for confidential review base templates -

  • Assign to UpRaise admin

  • Assign to reviewer's manager

  • Assign to UpRaise Admin - shared with Team Member

Template details

Below fields are available in template details -

  • Name - Template name

  • Description - Description of the template with additional details about the template.

  • Distribution mode - Based on selected modes, relevant templates will be listed when you select triggers in Form rule automation.

  • Other settings - define whether you want the Feedback and issues tab to appear within the form response page for the reviewer’s reference. Having these tabs enabled will allow reviewers to have feedback and Jira issue information handy for the team member they are reviewing.

Add these template details and click on Next.

Add questions

Add a relevant questions and sections in the template. The form creation process is the same for every template; the difference is only for form users.In this form, there is only one type of user involved that is reviewer. Read more about questions and sections in the template and save the template. You can save it and distribute it later, or distribute it right away.

Distribute

As explained in Manage distributions. The process of distribution is the same for all types of templates.

UpRaise allows to import an Excel file that contains the list of reviewers. Once can easily prepare the file and export directly instead of manually setting up users to distribute forms. Read Bulk importing users to a distribution via Excel upload.

Once the entire setup is done, one can save, schedule, or distribute the form. read Manage distributions | Distribute, Save as draft or Schedule distribution.

View responses

On the view responses page, admin/HR/current assignee of the form (whoever distributes the form) can keep an eye on the form statuses. It is also possible to export the distribution data to Excel and use that raw data for internal reporting.

As shown in the above image, there are three tabs on the View Responses page.

  • Forms - Displays all the forms in the distribution

  • Reports - Generate a report with the given distribution right away, instead of navigating to the reports section

  • Details - All the distribution details

Forms

  1. As shown in the above image, Forms tab is all about the forms and their information and actions. From this tab, the top chart shows the statuses of the forms.

  2. Filters to search for a specific set of users.

  3. Plus icon allows adding more user pairs to the ongoing distribution.

  4. Actions (…) beside the plus sign have actions like Export to Excel, Start/stop accepting inputs, and Extend due date. These options work at the distribution level. Below that, provided is the list of users added to the form.

  5. Every form is displayed with its current status, scores, assignee, etc.

    1. Green pencil icon dictates that form is still accepting input. When the form is closed, red pencil icon will be displayed, and it won’t accept any input.

  6. Actions (…) has options like Export to Excel, Start/stop accepting input, Delete, Print. This will be available for each team member and work at user level only.

  7. View option to open and view the individual form.

Reports

This tab has a two types of reports. Clicking on report generates a report for the current distribution. This is a handy way to generate a report instead of navigating to the Reports section in the app. Read Reports.

Details

This section is quite self-explanatory. This tab contains all the distribution details added while creating a distribution. A handy way to check the distribution setup.
